Migrating to New Patient Forms

If you missed the opportunity to try out the new patient forms, the ability to opt in to using the new patient forms solution is being re-enabled. Also, new functionality that will accompany the migration to the new patient forms solution will be rolled out in three phases. Then, after the opt-in period ends, all offices will be migrated to the new patient forms solution.

Opt-in Phase 1 (on 6/20)

On or after the start of phase 1, if you opt in, your legacy forms (questionnaires and consent forms) are converted to the new forms solution. For those of you who already started building new forms (or even started having patients complete the new forms) prior to this phase of the release, the legacy forms are available as templates that you can use to create new forms.

Notes:

  • Do not upgrade if you must use bilingual consent forms because currently only the English forms are converted.
  • Do not upgrade if you must use a kiosk that cannot scan a QR code to access the online portal because currently the push-to-kiosk workflow is not available.
  • Active forms that are converted have a “Published” status. Inactive forms that are converted have an “Unpublished” status.
  • With the legacy forms system, the online portal has a Terms of Use that patients have to acknowledge to use the site, but that form does not exist with the new online portal. If you want to provide a similar consent form with the new portal, you have to create a form and publish it in the new forms system.

If you are ready to convert your legacy forms and start using the new forms, on the Patient Forms page (go to Settings > Patient Forms), click Try New Forms, and then click Upgrade Now.

Note: Until phase 3, you may see duplicate forms in a patient’s record. For instance, if an existing patient has completed or is due to complete a “Dental History” form, you will see a “Dental History” form that corresponds to the legacy system and a “Dental History” form (converted from the legacy system) that corresponds to the new system.

Opt-in Phase 2 (on 7/2)

If you opted in (upgraded) since the start of phase 1, during phase 2, data from any legacy form submissions are migrated to the new forms system because it is possible for patients to complete legacy forms using links that were provided prior to you upgrading to the new forms system.

Note: Links that you send to patients to request the completion of forms are valid for seven days.

Opt-in Phase 3 (on 7/16)

If you opted in (upgraded) since the start of phase 1, when phase 3 starts, any override that was entered for a legacy form in a patient’s record is respected. Before now, it was possible to have a duplicate form in a patient’s record (one legacy and one new); however, those are now merged into one entry in the patient’s record; and if there was an override for the due date for the legacy form, the override is applied to the new form.

Automatic Migration (on 8/6)

All offices that have not opted in to using the new patient forms solution are migrated automatically.

Note: If you use bilingual consent forms, because only the English forms are converted, save a copy of the non-English text for your consent forms BEFORE 8/6 so you can reuse that text once bilingual forms are available.
